After each promotion an Evaluation Sheet must be filled out and returned within 3 days of that promotion or payment will be delayed. If I do not receive a sheet, I will not be able to pay you
for the promotion until I receive the sheet in my hands.
If you ever have a change of address or phone number, you must contact me immediately to update your file in the system,
otherwise there might be a delay in pay.
